Trainee AgreementFull version

I understand that USTrainee provides DS-2019 forms but cannot guarantee J-1 visa issuance.
The J-1 Visa is non-immigrant and not a path to permanent residency or work visa.
I will not apply for a visa status change during the program.
I confirm all submitted information is accurate and complete.
I will follow all J-1 rules as per U.S. State Department regulations.
I understand USTrainee is not responsible for employment disputes caused by my own conduct.
I will participate only during DS-2019 dates; the grace period is not for training.
I confirm I have $2,000+ to support myself during the program.
I acknowledge insurance is mandatory and valid only during program dates.
I release USTrainee from liability for any injury, travel issue, or force majeure event.
I indemnify USTrainee for any damage I may cause during the program.
I authorize USTrainee to share my application data for placement and housing support.
I understand that my personal information will be stored and handled securely, used only for program purposes, and protected in accordance with U.S. data privacy laws. Any unauthorized sharing or misuse may result in disqualification or legal consequences.

Company AgreementFull version

We agree to host a J-1 Trainee in accordance with U.S. Department of State regulations.
We will provide a DS-7002 training plan and rotate the trainee through phases as planned.
We will not displace U.S. workers and will comply with all labor laws.
We agree to provide supervision, mentorship, and training in line with program objectives.
We understand USTrainee is the official sponsor and may conduct site visits and evaluations.
We will notify USTrainee immediately of any incidents or changes in the training program.
We understand and agree to protect all personal information of trainees that we receive, and will use it only for program-related purposes. Any violation may result in immediate suspension and regulatory penalties.

Agent AgreementFull version

I acknowledge I act as a facilitator and not as a sponsor or employer.
I will provide accurate and honest assistance to applicants.
I will not charge unauthorized fees or misrepresent USTrainee services.
I understand I may be audited and removed for violations.
I understand I may handle sensitive applicant data, and I agree to protect all personal information in accordance with U.S. privacy laws. Any leak, mishandling, or misuse may result in legal consequences and permanent ban from the program.
